Weekly research phases

Units and volumes recompute from your Step 1 concentration (1 mL BAC → 5.00 mg/mL · 50.0 mcg/unit).

Standard titration

Investigational CagriSema programs titrate both amylin and GLP-1 components (often toward ~2.4 mg of each weekly after multi-step escalation). This fixed equal-mass vial keeps a 1:1 ratio in every draw — total blend mg = 2 × each peptide mg.

Bacteriostatic water

1 × 3 mL bottle

1 mL BAC per vial × 3 vials = 3.0 mL total. Buy as 1× 3 mL bottle or 1× 10 mL bottle (whichever you stock).

12+ syringes

About one 100-unit syringe per injection for this schedule. Keep a few spares.

Alcohol swabs & sharps

~30 swabs · 1 sharps container

Wipe vial stopper and injection site each time. Dispose needles in a proper sharps container.

Reconstitution steps

  1. Allow the lyophilized 5 mg Cagri+Sema blend vial and bacteriostatic water to reach room temperature.
  2. Draw 1.0 mL bacteriostatic water (default for this guide).
  3. Inject BAC slowly down the inner wall of the vial — avoid blasting the powder cake.
  4. Gently swirl or roll until the solution is clear. Do not shake hard (foaming).
  5. Label with recon date and concentration (e.g. "5.0 mg/mL total — 1 mL BAC").
  6. Refrigerate at 2–8 °C (35–46 °F), protected from light. Prefer using a reconstituted multi-dose vial within ~28 days with sterile technique.

Storage

Lyophilized

Keep dry powder cold and dark when possible (−20 °C / −4 °F preferred for long-term research stock).

Reconstituted

Refrigerate at 2–8 °C (35–46 °F). Avoid freeze–thaw of the working solution. Prefer finishing within ~28 days.

  • ·Write recon date and BAC volume on the label.
  • ·Protect from light during storage.
  • ·Discard if cloudy, particulates, or odd odor appear after reconstitution.

Protocol notes

  • ·Tables use TOTAL BLEND milligrams — fixed 50% cagrilintide / 50% semaglutide by mass.
  • ·You cannot independently titrate cagri vs sema from a fixed equal blend without separate vials.
  • ·Research use only — not approved combination product.

Research context & considerations

CagriSema combines long-acting amylin analogue cagrilintide with GLP-1 agonist semaglutide. Large outcome programs have reported substantial weight-loss signals for the dual approach. Research-market dual vials are not clinical drug product.

  • ·GI effects can be additive when stacking amylin + GLP-1 pathways.
  • ·GLP-1-class label cautions (e.g. MTC/MEN2 discussions on approved products) remain relevant framing.
  • ·Not FDA-approved as a research-vial consumer medicine.
  • ·Pregnancy and pediatric use out of scope.
